In accordance with Article 13(1) and (2) of the General Data Protection Regulation of April 27, 2016 (hereinafter referred to as: GDPR) this is to inform you that:

1. The controller of your personal data is EMC Instytut Medyczny SA with its registered office in Wrocław (54-144) at Pilczycka 144-148.
2) The contact person for personal data protection matters is the Data Protection Officer, available at the following e-mail address: iod@pentahospitals.pl.
3) Your personal data in the form of name, surname, telephone number, e-mail address will be processed as part of the registration for the Penta Hospitals International Leadership Conference 2025 Standing Out pursuant to Article 6(1)(a) of the GDPR.
4) Your personal data in the form of name, surname, telephone number, e-mail address will be processed by authorised employees of the data controller. The recipient of your personal data in the form of name, surname, telephone number, e-mail address will be the company organising the conference on the basis of the data protection agreement.
5) Your personal data such as name, surname, telephone number, e-mail address will not be transferred to third countries or international organisations.
6) Your personal data in the form of name, surname, telephone number, e-mail address will be stored for the period necessary to achieve the purpose specified in point 3 of the clause, or until you withdraw your consent. The data will then be deleted.
7) You have the right to access your data and the right to rectify, delete, limit processing, the right to transfer data, the right to object, and in the event that the basis for processing is consent – the right to withdraw consent at any time without affecting the lawfulness of processing based on consent before its withdrawal.
8) You have the right to lodge a complaint with the supervisory authority if you believe that the processing of personal data in the form of an image concerning you violates the provisions of the GDPR.
9) Providing your personal data such as name, surname, telephone number and e-mail address is voluntary. Failure to provide data such as name, surname, telephone number, e-mail address will result in the inability to register and participate in the Penta Hospitals International Leadership Conference 2025 Standing Out.
10) Your personal data such as name, surname, telephone number, e-mail address, image will not be processed in the form of profiling.
